Sever's Disease Etiology / Epidemiology / Natural History
- Apophysitis at the calcaneal insertion of the Achilles tendon.
- Generally boys age 12 or girls age 11. Related to adolescent growth spurt with weak ankle dorsiflexion and tight gastrocsoleus complex.
- May be overuse injury caused by repetitive microtrauma to the trabecular metaphyseal bone. (Ogden JA, JPO 2004;24:488).

- A/P and lateral calcaneous: may be normal or demonstrate fragmentation of the calcaneal apophysis. (Volpon JB, Arch Orthop Trauma Surg 2002;122:338)

- Plantar flexion stretching, dorsiflexor strengthening, foot orthosis vs heel lifts.
- Consider cast immobilization in recalcitrant cases.
