Pelvic External Fixation Indications

  • hemodynamically stable or unstable pt with A/P compression type II and III,
  • lateral compression type II and III
  • vertical shear
  • combined instablility fractures. (Poka A CORR 329:54;1996)

Pelvic External Fixation Contraindications

  • Floating iliac wing
  • Ipsilateral acetabular fx
  • Arterial bleeding

Pelvic External Fixation Planning / Special Considerations

Pelvic External Fixation Technique

  • Pre-operative antibiotics, +/- regional block
  • General endotracheal anesthesia
  • position. All bony prominences well padded.
  • Examination under anesthesia.
  • Prep and drape in standard sterile fashion.
  • Irrigate.
  • Close in layers.
  • Technique-skin incisions at 90 degrees to iliac crest, use guide sleeves, 5mm or larger blunt 1/2 pins, 2 converging pins in anterior 1/3 of iliac wing, first pin 2cm posterior to ASIS, feel center of iliac crest with guide, aim towards greater troch, drill cortex only then hand tighten half pins aiming towards greater troch frame must allow access to abdomen and allow for swelling,
