2 days post-op
- Remove dressing. Do not remove steri-strips. Place bandaids over incisions.
- WBAT with crutches as needed.
- Begin gentle AROM exercises
- Hip isotonics
- Quad/Ham string Sets
- SLR
- Crutch Training
- Modalities for swelling/pain
- Thigh high Ted-hose for universal compression.
- Begin patellar mobilizations (Medial glides and Medial tilts)
Phase I (2 days – 1 week)
- VMO/Quad retraining
- WBAT, may discontinue crutches when Quad function / knee control has returned. Generally 1-4weeks.
- Continue modalities for swelling/pain
- Stationary bike for ROM (no resistance)
- Hip isotonics
- Continue thigh-high Ted hose for universal compression
Phase II (1 – 3 weeks)
- VMO/Quad retraining / strengthening
- WBAT, may discontinue crutches when Quad function / knee control has returned. Generally 1-4weeks.
- Start weight activities (hip sled, hip machine, ham curls, heel raises, TKE’s, Ball wall squats (VMO strengthening)
- Closed kinetic strengthening exercise, high-reps, low weight
- Eccentric strengthening (Step downs, TKE’s)
- ROM (0-120º)
- Slowly wean from thigh-high Ted hose provided swelling has completely resolved
Phase III (4 – 6 weeks)
VMO/Quad Strengthening
Full ROM
Work into sport specific training if symptom free, good strength, no swelling
Provide home exercise program.
