Hypothermia Clinical Evaluation
- Cold +/- wet. Depending on severity may be shivering, confused, or unconscious.
Hypothermia Xray / Diagnositc Tests
- Check vital signs. Decreased pulse indicates moderate to severe hypothermia
- Measure core temperature
- Consider continuous oxygen saturation and EKG.
Hypothermia Classification / Treatment
- Mild (Core temperature: 32°–35°C (89°–95°F): Confusion, shivering. Treatment: remove from the cold environment; wet clothing, removed. Active warming using exogenous air-warming blankets.
- Moderate (Core temperature: 28°–32°C (82°–89°F): Impaired judgment and apathy, shivering stops, muscle stiffness. Treatment: remove from the cold environment; wet clothing, removed. Active warming using exogenous air-warming blankets and active internal rewarming with IV fluid warmed to 40°C to 43°C (do not use lactated Ringers-cold liver cannot metabolize lactate).
- Severe (Core temperature: < 28°C (82°F): Loss of consciousness, decreased pulse, decreased respiratory rate, coma. Treatment: remove from the cold environment; wet clothing, removed. Active warming using exogenous air-warming blankets and active internal rewarming with IV fluid warmed to 40°C to 43°C (do not use lactated Ringers-cold liver cannot metabolize lactate). Consider extracorporeal rewarming-peritoneal dialysis / chest tube irrigation.
